Program Description

*Provides pregnant women and children 0-36 months care coordination and home visits. Developmental assessments. Parenting Education and support. Breastfeeding & nutrition education. Educational materials about prenatal care, birth and infant care. Information and referral to community services and programs. *English/Spanish language spoken. *After hours appointments are available.

Eligibility Requirements and Important Information

Any female resident of Manatee County who is pregnant, as well as infants and children up to age three are eligible. Community and self-referrals are accepted. Participation is free and voluntary, regardless of age, race, ethnicity, religious affiliation, marital or immigration status.
