Eligibility Requirements and Important Information

Call CMS Clinical Eligibility Hotline 855-901-5390 for eligibility screening review. Parent survey is completed to determine eligibility. If eligible family must call MMA Choice counseling line 877-711-3662 to request enrollment if on Medicaid. If on KidCare enrollment in CMS plan is automatic. Once enrolled in CMS Health Plan a case manager will be assigned to assist with care access.
