Program Description

*The Adult Health program provides assessment and intervention, health education and referrals. *Physicals for employment and school entrance. *Adult immunizations and laboratory titers. *Tuberculosis skin testing. *Screenings for chronic disease (hypertension, diabetes, heart disease). *Diabetes prevention and self management education. *Injury prevention education (all ages). *Tobacco prevention, education and cessation referral. *Medications for Epilepsy, Diabetes, and HIV provided through special state programs. *Appointments are made by phone. *English/Spanish/Haitian Creole language spoken. *Language line translator service available for other languages.
