How This System Works
   
   

The Whole Child Project Manatee Database System is a free community resource that allows families like yours to ensure that their children aged 5 or less are receiving all the services they need to be happy, healthy and successful.

To accomplish this goal, your are asked to complete a Whole Child Profile. A Whole Child Profile is a series of questions which help identify services that your child may need. You do not need to put your name on your profile. Once a profile has been completed, this system will analyze your profile and will provide you with a list of services that your child or family may find useful, along with an explanation of what this service entails.

Based on your profile analysis, you will also receive a sorted list of providers in your area that can help you receive these services.

You are encouraged to select a list of providers that you think may be able to help you receive the services that your family needs. After you make your selections, each provider you selected will receive a copy of your profile and then a Whole Child Plan will be developed exclusively for you. Each provider you selected will automatically receive a copy of your profile.

You can also save your Whole Child Plan online, keep track of your progress over time, and easily release your profile information to any provider you choose. Our goal is to make receiving services for your children as simple and hassle-free as possible, all while ensuring your privacy and freedom of choice.

If you would like to use this system or receive services from providers, Whole Child Advisors are available and free of charge.

The Welcome Page
   
   

When you first connect to www.wholechildmanatee.com you will arrive at the Welcome Page. From here you can select which language you prefer to use to:

1. complete a Whole Child Profile,
2. return to your existing Whole Child Plan,
3. contact an advisor to assist you,
4. or access the help file.

You may return to this page at any time by clicking on “HOME” in the yellow bar at the top of your web browser. You may also get help by clicking on “HELP” in the yellow bar at the top of your web browser.

If you would like to change the language of the system, click on either the word “Español” or “Inglés.”

If you would like to complete a Whole Child Profile, simply click on the "Complete A Profile" link. You will be taken to the Whole Child Profile page. Click here if you want to skip ahead to that point in the help file.

If you would like to return to your existing Whole Child Plan or Update your most recent Whole Child Profile, click on "Returning Visitor?" and you will be taken to a screen that asks for your “User Name” and “Password”. To protect your privacy, you must enter your “User Name” and “Password” exactly as you entered it on your profile (use same upper or lower case letters) to recover your Whole Child Plan.

The Whole Child Profile Page
   
   

The Whole Child Profile Page is where you answer questions that provide the system with information about the needs of your family. This page contains questions about your family and spaces for you to answer questions about yourself.

Your responses to the questions are used to determine the potential needs of your children.

The information requested about where you can be reached will be given to service providers so they can contact you (if you request them to do so). Please provide as much information as you are comfortable with.

On this page you will also select a “User Name” and “Password”. Your User Name and Password act as your key to the information you have provided in your profile and allow you to do two things:

1. They allow you to return to your Whole Child Plan at a later date.

2. They allow you to provide an electronic copy of your Whole Child Profile to any service provider or advisor participating in the Whole Child Manatee Database system at any time you wish in the future.

You are not required to provide a User Name and Password. However, if you do not provide a User Name and Password you will not be able to provide copies of your Whole Child Profile to service providers or advisors after you complete your Whole Child Plan.

The Whole Child Profile

If you would like to complete a Whole Child Profile, follow the step by step instructions below:

Read each question and answer YES or NO by clicking on the circle directly to the right of each question. If you prefer, you may use your TAB key to move between questions. You do not have to answer each question. If a question is not applicable, do not answer that particular question. If you use the BACKSPACE key while answering the Yes or No questions, you will be taken back to the home page. To return to the profile, click the "Forward" button on your web browser.

Some questions allow you to type responses. To type a response, click on the empty text box below the question and type your answer.

Fill in the blanks below the word “Adults.” The person completing this profile should enter their information into the horizontal row labeled "Adult 1." You must provide your employment status to continue.

Fill in the blanks below the word “Children.” You must provide the date of birth for at least one child below the age of five to continue.

Fill in the information below the word “Contact Information." You must provide your zip code to continue. Please provide contact information so that service providers and advisors may contact you. You will only be contacted if you specifically request it. Providing your contact information does not mean that you will automatically be contacted. You will only be contacted if you specifically request it.

Indicate your family income by clicking on the appropriate circle to the right of the words “Family Income.” You must provide your family income to continue.

Enter a user name and password. Your user name and password serve two purposes:

1. They allow you to return to your Whole Child Plan at a later date.

2. They give your permission to allow service providers to view your Whole Child profile.

You may use whatever words you would like for your user name and password, but make sure you will be able to remember them.
We highly recommend entering a user name and password. By doing so you give yourself the option to return to your Whole Child Plan or, if you so desire, share your information.

If you are informed that your user name and/or password has already been chosen, please select another user name or password.

Please print your Whole Child Profile by clicking on the Print button located at the bottom of the page. Printing your profile provides you with a record of your answers along with a handy copy of your user name and password.

When you have finished filling out the profile, CLICK ONCE on the Submit button at the bottom of the page.

If you decide that you do not want to submit your Whole Child Profile, click on the Cancel button and close your browser window. When you do this your information will be permanently erased.

Once you have finished entering information into the profile, the system will analyze your profile and the results of your profile analysis will be displayed in a few seconds, depending on the speed of your internet connection.

Some services may be difficult to obtain. That's why, free of charge, Whole Child Advisors are made available for you. If your profile indicates that you may have a need for one of these services, you will be directed to speak to a Whole Child Advisor who will assist you.

All families using the Whole Child Manatee system are welcome to speak with an Advisor if they would like.

The Whole Child Profile Analysis Page shows you the services you may need, along with a description of those services.

Please read these descriptions so that you understand what each service can provide. You may determine that a service is not what you require. The analysis provided to you is solely based on your profile responses, and you are free to disregard any service needs that you feel are inappropriate or unnecessary.

If you would like to see a list of service providers for a particular service, click on the checkbox directly to the right of the service name. You must select at least one service type to continue.

You are encouraged to print this page for your records. To print the page, simply click on the word “Print” at the bottom of your page, or use the print option of your web browser.

When you are finished reviewing your Whole Child Profile analysis, please click on the “Display Provider Details” button at the bottom of the page. You may return to this page by clicking back on your web browser.

The provider details page locates service providers in your community that can help you receive the services that were identified during your Whole Child Profile Analysis.

Service providers are grouped by services that they provide. You can tell what services each provider offers by looking at the green bar above their listing which reads "Services Available Through This Program."

Each service provider's name will be listed in a yellow bar. Below the yellow bar is a summary of information for this particular program.

If you would like to view detailed information for a service provider program, or if you would like to print out a single program summary, click on the program you wish to see.

For example, if you would like to view more information about the Healthy Kids program you would click on the words "Healthy Kids."

Each time you click on a program name, a separate window will be displayed containing all the details available on the program you selected. You may open as many of these windows as you like. You may also print one of these windows by clicking on the Print button located at the bottom of each window.

If you would like to see a map of the area around a particular service program provider, click on the program's address. Note that some programs do not provide street addresses and this feature will not work for programs that only provide post office boxes.

You may click on the back button, or the floating "Change Services" button, to return to the analysis page and select different providers. This process may be repeated as many times as you wish.

   
Selecting Service Providers
   
   

When you have finished reviewing the information about service providers, you are encouraged to select programs that offer services that you think will meet your needs.

To indicate that you would like to receive services from a program, click on the box to the right of a program's name. These boxes are also located in the yellow bars. You may turn these selection boxes on and off by clicking them a second time. Your choices are not submitted until you press the "Submit" button.

By selecting a service program, you are notifying the person responsible for this program that you would like to learn more about their services. You will also automatically provide this program with a copy of your Whole Child Profile and contact information so that they better understand your particular needs and have your permission to contact you. Remember that a provider will not know how to reach you if you did not provide your contact information in your Whole Child Profile.

Please select only those providers you wish to speak to.

When you have finished selecting service programs that meet your needs, click the “Submit” button at the bottom of the page. You will then receive a summary of your selections that you may print.

   
The Whole Child Plan Page
   
   

The Whole Child Plan page is a summary listing of all service providers you selected on the previous page. These listings are called Service Requests.

The purpose of the Whole Child Plan is to provide you with a complete list of service providers to address all of your selected needs. Please print this page by clicking on the Print button located at the bottom of this page.

Each Service Request listed on your Whole Child Plan appears below a green bar that indicates what service need this program can help you with.

The Service Request record contains all the information available on this program. It also provides two new areas that were not visible on the provider details page:
1. a status section and
2. a notes section.

Status

The first new area is a section labeled Status. If you entered a User Name and Password on your Whole Child Profile, you may return to this page at any time and from any computer that has a web browser. Service providers will update the status of your service request so that you know:

1. when appointments have been scheduled
2. if you declined an appointment
3. if your service request was completed.

"Notes to you from you Advisor / Provider"

The "Notes to you from you Advisor / Provider" section allows service providers and Advisors to leave you notes. These notes will only be viewable if you return to your Whole Child Plan. Please let your Advisor or service provider know if you are using your Whole Child Plan online so that they may provide helpful information to you in these areas.

When you have finished reviewing your Whole Child Plan, please click on the “Next Steps” button located at the bottom of the page. Alternatively, you can use your browser's "back" button to return to the previous page and select additional service providers, if so desired.

Returning To Your Whole Child Plan
   
   

If you provided a User Name and Password on your Whole Child Profile you have the ability to return to your Whole Child Plan at any time and from any computer with a web browser.

Simply connect to the Welcome Page and click on the “Returning Visitor?” words at the left hand side of the screen. You will be taken to a page where you may type in your User Name and Password.

Enter your User Name and Password exactly as you provided them. You must use the proper upper and lower case characters.

If you have forgotten your User Name and Password, please simply create another profile.

You may also use your User Name and Password to release your profile and contact information to service providers or Advisors. Providing this information can help service providers and Advisors address your needs more efficiently.

To provide your Whole Child Profile and Contact Information to a service provider or Advisor access to your Whole Child Profile and Contact Information, simply tell them your User Name and Password.

If you have forgotten your User Name and Password, please simply create another profile.
